Profiles use the CMS Provider Data Catalog: Provider Information, Health Deficiencies, Fire Safety Deficiencies, Survey Summary, Ownership, Penalties, and claims-based and MDS quality measures. CMS certification numbers link the datasets.

How comparisons work

State medians use facilities with reported values. County medians are withheld when fewer than five facilities report the measure. Missing and suppressed values are not zero. “Serious deficiencies” in this FL directory includes scope/severity G–L; immediate jeopardy is J–L. Fines and citation counts cover the records supplied by CMS, not a lifetime total. Special Focus candidates are distinguished from participating facilities in profile labels.

Updates and limitations

The updater checks state, unique identifiers, feed completeness and snapshot consistency before publication. A failed update should leave the last validated release available. These checks do not independently verify clinical care. CMS data can lag events, and ratings do not predict an individual resident’s experience. Article review dates are separate from CMS refresh dates.
